The Bridge House Homeless Outreach Program provides aggressive, flexible, outreach services to individuals with a psychiatric disability and substance abuse problems who are homeless or at risk of becoming homeless, or who currently live in shelters or other locations not intended for human habitation. Case Management ServicesCase management services are provided to assist clients in gaining access to the clinical, medical, social, educational, rehabilitative, vocational and/or other services that are essential to achieving optimal quality of life and community living. Case managers work with clients collaboratively in identifying needs and developing individual service plans. As necessary, case managers arrange for services, transport clients to programs, regularly check on progress, monitor the impact of services, and advocate on behalf of clients. Case managers also provide ongoing monitoring and assessment of each client's service plan to assures that goals are being achieved. The Drop-in CenterThe Homeless Outreach program has a drop-in center in downtown Bridgeport, which provides "one-stop shopping" for homeless individuals. Services provided at the drop-in center include, but are not limited to, the following:
